HOME HEALTH CARE

RN Per Diem

Provides professional nursing care for assigned patients in their home. Evaluates, plans, and makes appropriate changes to the home care nursing process. Assesses the patient's condition and the home environment. Instructs patient and family members on health care

Home Health Aide

Provide routine, personal healthcare, such as bathing, dressing, or grooming, to elderly, convalescent, or disabled persons in the home of patients or in a residential care facility.

Home Care Scheduler
