RN Per Diem

Provides professional nursing care for assigned patients in their home. Evaluates, plans, and makes appropriate changes to the home care nursing process. Assesses the patient's condition and the home environment. Instructs patient and family members on health care.

ESSENTIAL DUTIES AND RESPONSIBILITIES

1. ASSESSMENT

  • Plans to meet the needs of each client and provides direct continuous care through the application of the nursing process based upon theory, research and approved Nursing and institutional standards.
  • Provides ongoing assessment of client as per service routine and according to client acuity.

2. PLANNING

  • Coordinates client care activities, based on established priorities, including teaching, counseling, and discharge planning, and directs/delegates appropriately to homecare team.
  • Prepares and utilizes an individual nursing care plan based on the assessment of assigned clients.

3. IMPLEMENTATION

  • Assesses client needs and provides clinical interventions, recognizing the developmental age and psychosocial needs of the clients; understands the cognitive, physical, emotional, and chronological maturation process.
  • Is knowledgeable about medications, IV solutions, treatments and tests.
  • Is knowledgeable about client’s conditions most commonly encountered.
  • Provides nursing skills based upon competencies for specific units.
  • Performs technical procedures and administers medications safely and according to current practice.
  • Adheres to safety and infection control measures and recognizes and reports hazards.
  • Documents nursing actions, client responses and other nursing observations according to established policies and procedures on appropriate nursing records.
  • Adheres to Home Care Services standard of nursing practice and policies and procedures.

4. EVALUATION

  • Evaluates client needs for, and response to, care rendered and applies sound nursing judgment in client care management decisions.
  • Revises plan of care as appropriate.
  • Evaluates client’s responses to teaching and documents this evaluation appropriately.

JOB QUALIFICATIONS
Licensed or otherwise duly authorized to practice as a professional nurse in New York State. Current CEU’s. Minimum two years experience working in a home health care environment. Valid NYS driver’s license.
